DODD & Medicaid Waiver Services
Walker Wellness Homes provides person-centered DODD and Ohio Medicaid waiver services designed to support eligible individuals with developmental disabilities in building independence, participating in the community, and achieving their personal goals.
Services are provided according to each individual’s authorized service plan, assessed needs, and approved supports.
Our Waiver Services
Homemaker Personal Care (HPC)
Homemaker Personal Care supports eligible individuals with daily living activities and the development of greater independence.
Services may include:
Personal care and hygiene support
Meal preparation
Household routines
Skill development
Community participation
Assistance with daily activities
Support with goals identified in the individual’s service plan
Home Delivered Meals
Walker Wellness Homes may provide Home Delivered Meals to eligible individuals when the service is authorized through the individual’s waiver or service plan.
Meal services are provided according to program requirements and individual needs.
Non-Medical Transportation (NMT)
Non-Medical Transportation helps eligible individuals access approved community destinations and activities.
Transportation may include:
Employment
Day programs
Community activities
Errands
Appointments
Other authorized destinations included in the individual’s service plan
Participant-Directed Homemaker Personal Care
Participant-Directed HPC allows eligible individuals to have greater choice and involvement in how their authorized supports are delivered.
Services are based on the individual’s approved waiver, authorized service plan, goals, and support needs.
Who May Benefit
DODD and Medicaid waiver services may be appropriate for eligible individuals who need support with daily living, personal care, independence, community access, transportation, skill development, or other authorized needs.
Eligibility for waiver services is determined by the appropriate county board, Medicaid program, or other authorized agency.
